Primary Care Networks

Primary Care Networks (PCNs) form a key building block of the NHS Long Term Plan. While GP practices have been finding different ways of working together over many years, the NHS Long Term Plan and the New Five-Year Framework for the GP contract (both published in January 2019) put a more formal structure around this way of working.

Since July 1st 2019, all GP practices across West Cheshire have come together in networks each covering populations of approximately 30-50,000 people.

In West Cheshire, the seven Primary Care Networks bring together the same groups of practices as the GP Clusters – which means working relationships are already well-developed.

Each Primary Care Network has appointed a local GP (or GPs) as Clinical Director.

Chester East Primary Care Network consists of:

  • Heath Lane Medical Centre
  • Boughton Medical Centre
  • Park Medical Centre
  • Upton Village Surgery

Chester South Primary Care Network consists of:

  • Lache Health Centre
  • Handbridge Medical Centre
  • Western Avenue Medical Centre

Chester Central Primary Care Network consists of:

  • The Elms Medical Centre
  • Fountains Medical Centre
  • Northgate Medical Centre
  • Northgate Village Surgery
  • Garden Lane Medical Centre

Clinical Director – Dr Aidan Magrath (The Elms Medical Centre) and Dr Rebecca Banks (Northgate Village Surgery)
