Our Programmes

Developing Primary Care

The ambition of the Developing Primary Care programme is to transform the way primary care is delivered, providing timely access and continuity of care for patients (particularly those with complex conditions) and to support practices to take on a crucial role within the integrated health and social care teams.

These multi-disciplinary teams of GPs, community nurses and social workers support the effective delivery of co-ordinated care in West Cheshire.

Urgent and Emergency Care

The vision in West Cheshire is to deliver the best quality and most appropriate urgent or emergency care to those who need it and, once care is complete, to discharge them safely to their place of residence. This means care delivered closer to home, fewer avoidable admissions to hospital and shorter waiting times for treatment.

Starting Well

Our Starting Well ambition is to support babies, children, young people and their families in West Cheshire to have the best start in life, to promote self-care and to reduce reliance on hospital care.

Planned Care

Our Planned Care programme focuses on the commissioning of services for people with long-term conditions, as well as planned hospital care such as outpatients and operations. Key areas of work include reducing unnecessary care variation and improving patient outcomes.

Mental Health and Learning Disabilities

Many people's lives will, at some point, be touched by mental ill-health. Key areas of focus for our Mental Health and Learning Disabilities programme include ensuring that people get the care that is right for them and helping to reduce the number of hospital admissions.

Medicines Management

It is vital that people are able to access the right medication at the right time. Our Medicines Management programme works hard to achieve this for local patients as well as focusing on improving the local offering by supporting and identifying medicines reviews and by helping to reduce medicines waste.

Complex Care

The local complex care / continuing healthcare service operates as part of a shared service which covers five clinical commissioning groups across Cheshire and Wirral – including NHS West Cheshire Clinical Commissioning Group.
